Health Disparities Collaboratives

The Health Disparities Collaboratives (HDC) are an exciting initiative of the Bureau of Primary Health Care (BPHC), in cooperation with the Institute for Healthcare Improvement (IHI). This initiative utilizes a new approach towards quality improvement to accelerate learning, innovation and change. It is designed as a multi-year project to improve care and outcomes for chronic diseases.

The initiative works on a quality improvement model implemented in health centers by a team of clinical and administrative staff. Measures of success already show improved self-management skills for over 100,000 patients across the country. The organization of the BPHC Health Disparities Collaboratives is in five regional clusters. The following states and territories are located in the Pacific West Cluster, managed by staff housed with the California Primary Care Association: Alaska, American Samoa, Arizona, California, Guam, Hawaii, Idaho, Nevada, Oregon, Palau, and Washington. Currently, the Collaboratives are in the fifth year of focus on chronic conditions and other related topics, such as asthma, cancer, cardiovascular disease, depression, diabetes, diabetes prevention, prevention, as well as finance/redesign.

Calfornia Quality Improvement Collaborative:

The California Quality Improvement Collaborative (CAQIC) is an initiative of the California Primary Care Association (CPCA), in cooperation with the California Healthcare Foundation (CHCF). This initiative utilizes the same quality improvement models and key outcome/process measures as the BPHC National Health Disparities Collaboratives, but for clinics and health centers in the state of California not funded by the BPHC.

Currently, the CA QI Collaborative is in the second year of focus on asthma and diabetes. For information about learning sessions and the application process for the California Quality Improvement Collaborative, please contact the staff listed on the Collaborative Staff contact page.
