As its name suggests, the Collaboratives are a collaborative effort among several partners, some national, some local/regional - including the Bureau of Primary Health Care (BPHC), the Institute for Healthcare Improvement (IHI), the Centers for Disease Control (CDC) and the five lead Primary Care Associations (PCAs) and other regional clinical networks. The lead PCA for each cluster is listed below:
