Collaborative Steering Committee

Activities in the Health Disparities Collaboratives are guided by a Steering Committee.  The Steering Committee members are representative of health centers, primary care associations, clinical networks and major partners, drawing on their disciplines and expertise.

The Pacific West Cluster Steering Committee is comprised of these professionals from Alaska, Arizona, California, Hawaii, Idaho, Nevada, Oregon, Washington and the Pacific Basin territories. Groups represented on the Steering Committee include: the Diabetes Prevention & Control Programs of these states/territories; Clinical Coordinators and Executive Directors from the State Primary Care Associations in the Cluster; the two Clinical Networks in the Cluster; Providers doing the Collaborative work in their Health Centers, and National Partners, such as the Migrant Clinicians Network, and Health Care for the Homeless.
