Delivering Evidence-Based Preventive and Chronic Illness Care

Participants will learn about the Chronic Care Model (CCM), a system change strategy to improve care for patients with chronic illness. The six components of the CCM are discussed in terms of what a practice team must do to implement each component. There is an overview of the literature of effectiveness if a system is successful implementing the CCM. The linkage between the CCM and the Patient-centered Medical Home is discussed.

$60 Per Connection for a CPCA Member or CCHC*
$75 Per Connection for a non-CCHC*
*These fees allow the organization to arrange for multiple participants to share one internet and phone connection. Additional registration fees apply for each additional connection used.
Cancellation:
To receive a full refund of your registration fee, CPCA must receive your written cancellation notice one week prior to the event. No refunds will be offered for cancellations after the cancellation deadline. Registrations may be transferred to another individual by written request up to the day of the event. No shows are responsible for payment of registration fees and are not eligible for a refund.
