Delivering Self-Management Support to Patients with Chronic Illness

Self-management support (SMS) is defined and compared to patient education. The nine core competencies to deliver SMS are discussed with a focus on brief action planning as a tool for practitioners. Opportunities for delivering SMS during the patient encounter are discussed. The presentation ends with resources participants can access to help with delivering SMS.

$60 Per Connection for a CPCA Member or CCHC*
$75 Per Connection for a non-CCHC*
*These fees allow the organization to arrange for multiple participants to share one internet and phone connection. Additional registration fees apply for each additional connection used.
Cancellation:
To receive a full refund of your registration fee, CPCA must receive your written cancellation notice one week prior to the event. No refunds will be offered for cancellations after the cancellation deadline. Registrations may be transferred to another individual by written request up to the day of the event. No shows are responsible for payment of registration fees and are not eligible for a refund.
