Improving health care among California’s health centers

The Health Center Check-up report measures the quality of care delivered by community clinics and health centers (CCHCs) in California. The data is publicly reported to support local, regional and state-wide quality improvement efforts.

CCHCs share a common mission to serve everybody who walks through their doors, regardless of ability to pay. In many California counties, CCHCs are responsible for providing a significant proportion of comprehensive primary care services to those who are publicly subsidized or uninsured. In these communities, CCHCs consistently demonstrate that they are the preferred provider of choice for high quality care.

Since 2007, the California Primary Care Association and its 12 partner regional associations have created a statewide system to collect, report, and analyze CCHC clinical data with a goal to improve and maintain the quality of care delivered to CCHC patients.  This statewide effort originates from the Aligning Quality Improvement in California Clinics for Meaningful Use (AQICC- MU) program created to help prepare California clinics for “meaningful use” of electronic health records to improve clinical outcomes and operational efficiency via four regional training and learning communities.

Users of this website can view CCHC data, aggregated at the regional association level and at the state level.

The past few years have been focused on increasing the use of registries in CCHCs to drive quality. Presently, not all patients in each clinic site are tracked in a registry. However, as we move toward implementation of Electronic Health Records (EHRs) in CCHCs, the data reported will provide a fuller picture of the care rendered. Clinical quality improvement will be a CCHC priority as they can efficiently monitor patients through the adoption EHRs.
