Patient-Centered Health Home (PCHH) transformation is essential but challenging work. The California Primary Care Association (CPCA) is here to support our member clinics by providing technical assistance and training in a wide variety of formats to meet their exact needs. The purpose of the initiative is to create an integrated strategy to support community clinics and health centers (CCHCs) to achieve patient centered health home recognition status, improve health outcomes, and position CCHCs for ongoing participation in primary care delivery under health reform.
The PCHH initiative is a flexible program designed to meet your clinic’s needs wherever you may fall on the readiness continuum. We offer multiple ways to engage with our highly qualified team of experts. Click on the links below to learn more about each level of support.
Our PCHH On-Demand Library features over 31 hours of PCHH pre-recorded training sessions covering a wide range of Patient-Centered Medical Home recognition topics, including: enhanced access, care team development, change management, and more. These sessions are web-based and available to an unlimited number of your providers and staff upon organizational subscription. Streaming content allows for flexible, convenient access and accommodates busy staff schedules. Click here to browse our library of course materials.
A mock survey is a preparatory review of your 2014 NCQA Standards application documentation, focusing on the 6 Must Pass elements. A trained reviewer will look through this documentation in detail, and provide you with feedback and areas where your health center can, or needs to, improve your application. Mock surveys provide an extra layer of reassurance before submitting your supporting documentation for review. The goal is for your health center to achieve the highest possible NCQA score.

“Had it not been for Amy Ham, our CPCA Practice Coach, review and suggestions, we would not have achieved our NCQA PCMH Level Three Recognition. Amy reviewed our documentation to ensure its appropriateness for the Standard, Factor and Element for which it was being submitted. Now that we are recognized, Amy continues to work with Clinicas de Salud del Pueblo to facilitate our PCMH Transformation process.”
