Patient-Centered Health Home (PCHH) transformation is essential but challenging work. The California Primary Care Association (CPCA) is here to support our member clinics by providing technical assistance and training in a wide variety of formats to meet their exact needs. The purpose of the initiative is to create an integrated strategy to support community clinics and health centers (CHCs) to achieve patient centered health home recognition status, improve health outcomes, and position CHCs for ongoing participation in primary care delivery under health reform.

Our PCHH On-Demand Library features over 31 hours of PCHH pre-recorded training sessions covering a wide range of Patient-Centered Medical Home recognition topics, including: enhanced access, care team development, change management, and more. These sessions are web-based and available to an unlimited number of your providers and staff upon organizational subscription. Practice Transformation Coaching

Our PCHH practice coaches provide tailored assistance to facilitate practice transformation and guide community clinics and health centers through the PCHH recognition process. Our PCHH practice coaches are experienced quality improvement professionals. They can support your community clinic or health center become and remain a recognized Patient-Centered Health Home. Practice coaches have a diverse set of skills and can work with your team on patient-provider empanelment, electronic health record reporting and template development, care team redesign, workflow redesign, sharing best practices, and much more. They will develop a custom coaching scope based on your needs and their vision will keep you focused on your practice transformation goals.

Had it not been for our CPCA Practice Coach, review and suggestions, we would not have achieved our NCQA PCMH Level Three Recognition.  Our coach reviewed our documentation to ensure its appropriateness for the Standard, Factor and Element for which it was being submitted.
