Care coordination and following up on referrals is an important feature of the medical home model. Today, health centers are strategically using data analytics from their Electronic Health Record (EHR) to improve clinical efficiency and enhance their care coordination models. This 60-minute webinar will highlight how to use EHR data to advance care coordination models through referrals, UDS clinical measures, etc.
LEARNING OBJECTIVES
- Learn how clinical and operation staff successfully collaborated on referral tracking and closing the loop;
- Identify promising practices around care coordination and transition of care workflows; and,
- Gain insight into introductory FQHC level utilization management for specialty referrals and ER frequent flyers.
