The Health Homes Program (HHP) is accountable for improving member outcomes by coordinating a variety of services - including palliative care. But what is palliative care, who can benefit from it, and what kinds of palliative care services are available to Medi-Cal managed care members? As direct care providers working closely with HHP members, CB-CMEs are poised to identify needs and connect eligible patients to palliative care services that will help them manage the physical, emotional, and spiritual suffering of their illness while they continue to receive disease-directed treatment from their primary or specialty care providers. This 75-minute webinar builds on a series of HHP-specific webinars poised to support community health centers participating in HHP as CB-CMEs to better assess and plan the right care for eligible beneficiaries.
LEARNING OBJECTIVES
- Understand what palliative care is and how it can help people with serious illness;
- Learn about the state requirement (Senate Bill [SB] 1004) that Medi-Cal managed care plans provide access to palliative care, and how those services are provided through a network of community-based palliative care providers;
- Understand the overlap in the Health Homes population and the SB 1004 palliative care population, and how to think about what services they need, and when; and
- Recognize the role that CB-CMEs can play in assessing the needs of people with serious illness and connecting them to palliative care providers as appropriate.

To receive a refund, all cancellations must be in writing and must be received by emailing training@cpca.org according to the following schedule:
- Cancellations received on or before 20 business days of the event will result in a full refund minus a 10% processing fee.
- Cancellations received on or before 10 business days of the event will result in a 50% refund minus a 10% processing fee.
- Cancellations received on or before 5 business days of the event will result in a 25% refund minus a 10% processing fee.
- No refunds will be offered for cancellations received after 5 business days before the event.
- Cancellations after the conclusion of the event are non-refundable.
- Substitutions are encouraged.
- “No Shows” are non-refundable.
