This program is an in-depth guide to NCQA Recognition requirements and the process for achieving PCMH recognition. Recognized practices and clinicians have proved the relevance of the NCQA PCMH standards to the Triple Aim. The 2017 standards emphasize a practice wide commitment to sustaining the PCMH transformation, demonstrating patient care and population health management, care coordination and ongoing quality improvement.

Practices are rewarded for integrating behavioral healthcare and addressing the impact of social determinants on patient needs. NCQA requirements are positioned to qualify for the Merit-Based Incentive Performance System (MIPS) Clinical Practice Improvement Activities (CPIA) and to support other MIPS requirements under MACRA. Join us for this thorough review of the PCMH 2017 requirements and the documentation needed to support an application for NCQA Recognition..
