Insights Into Implementation Strategy: Diabetes Prevention

This one hour webinar presentation will provide guidance to health care professionals interested in implementing a diabetes prevention strategy at their health center. This session will review the evidence for diabetes prevention and outline the actions that organizations can take to systematically identify and manage patients at risk for type 2 diabetes. The support and resources offered by the American Medical Association (AMA) to assist organizations in operationalizing diabetes prevention and the experiences of other health care organizations who have implemented a diabetes prevention strategy will be highlighted including best practices and key considerations. 

LEARNING OBJECTIVES
  • Describe the trends in type 2 diabetes and prediabetes and implications for clinical practice;
  • Review the research and guidelines that support systematically screening and managing patients with prediabetes with evidence-based treatments, such as the National Diabetes Prevention Program lifestyle change program;
  • Discuss key considerations for implementiation of a diabetes prevention strategy to improve population health; and
  • Outline the Learning Series opportunity and resources available from the AMA on diabetes prevention.

PRESENTER
  • Neha Sachdev, Director of Health Systems Relationships, Improving Health Outcomes, American Medical Association
    • As the Director of Health Systems Relationships in the Improving Health Outcomes department at the American Medical Association (AMA), Dr. Sachdev works with physicians and health systems to adopt evidence-based strategies to prevent cardiovascular disease. Prior to joining the AMA, Dr. Sachdev was a core faculty attending physician at the Virtua Health Family MEdicine Residency and a practicing family medicine and obesity medicine clinician. She worked with both graduate and undergraduate medical trainees and was a clinical instructor in the Department of Family Medicine at Sidney Kimmel Medical College of Thoams Jefferson University. She also servewd as the New Physician Trustee for the board of the New Jersey Academy of Family Physicians. After graduating from Sidney Kimmel Medical College, she completed her family medicine residency at Northwestern McGaw Medical Center through a HRSA-funded teaching health center program. She holds a Bachelor of Arts in Hispanic Studies and Health and Societies from the University of Pennsylvania.
  • Siga Visaitis, MHA, Physician and Health Systems Engagement Manager, Improving Health Outcomes, American Medical Association
    • As a Physician and Health Systems Engagement Manager in the Improving Health Outcomes department at the American Medical Association, Siga Vasaitis has over 6 years of experience in health care administration, over 5 of which has been in the FQHC setting. Siga works with hospitals and physician practices of all shapes and sizes to operationalize diabetes prevention strategies within their organizations. For over 5 years, she worked at Access Community Health Network, one of the largest FQHCs in the nation serving over 180,000 patients per year. During this time she worked in multiple roles in grant writing and majority of her time was spent in Clinical Operations as both a project manager and a Patient Centered Medical Home Manager. Throughout this time, she lead initiatives on improving access to care through an organization wide performance improvement initiative, worked towards improving communications with hospitals through Epic's EpicCare Link portal, and provided expertise in NCQA's patient-centered medical home model.
