Join CPCA and the Redwood Community Health Coalition for a three-part, web-based training and peer learning series for community health centers (CHC) interested in contracting as Community Based - Care Management Entities (CB-CMEs) within the state's Health Homes for Patients with Complex Needs (HHP) demonstration. This webinar series will provide CHC CB-CMEs with much-needed care coordination and case management foundational concepts by harvesting lessons learned from successful care management programs.

The second webinar in this series will focus on Standardized Care Planning and Health Action Planning. A core HHP service is comprehensive care management, which starts by collaborating with HHP members and their family/support persons to develop their comprehensive, individualized, person-centered care plan, referred to in HHP as a Health Action Plan (HAP). Each health plan will define their own HAP that CB-CMEs will complete and maintain. This webinar will review a sample care plan developed by RCHC that is currently utilized in six member clinics. It will also focus on the workflows and staffing structure used to revise the HAP as needed and reassess a member's health status, needs, and goals.

LEARNING OBJECTIVES
  • Understand what is included in and how to utilize a health-center-tested standardized care plan pilot tested by RCHC clinics;
  • Identify best practices and workflows for implementing the care plan from health centers utilizing the tool; and
  • Learn to recognize challenges CB-CMEs should expect with plan variation in data collection templates and in-congruent data sharing processes between plans.

Cancellation Policy:  No refunds will be offered for cancellations received after February 19, 2019.

To receive a refund, all cancellations must be in writing and must be received by emailing training@cpca.org according to the following schedule:

  • Cancellations received on or before 20 business days of the event will result in a full refund minus a 10% processing fee.
  • Cancellations received on or before 10 business days of the event will result in a 50% refund minus a 10% processing fee.
  • Cancellations received on or before 5 business days of the event will result in a 25% refund minus a 10% processing fee.
  • No refunds will be offered for cancellations received after 5 business days before the event.
  • Cancellations after the conclusion of the event are non-refundable.
  • Substitutions are encouraged.
  • “No Shows” are non-refundable.
