Join CPCA Staff and PCHH Practice Transformation Coaches during this 60-minute Q&A Office Hour where we will be answering your questions about starting and renewing PCHH Recognition efforts. Deciding to pursue PCHH Recognition is a huge investment and the practice transformation within a clinic can be difficult. However, we all know the value of becoming a medical home lies in increased patient care. Additionally, a health center on their journey with the new NCQA PCMH 2017 Standards will present their experiences. Come prepared with questions and to learn from your peer health centers!

LEARNING OBJECTIVES
  • Enhance understanding of NCQA PCMH 2017 Standards and new annual recognition process.
  • Improve ability to identify gaps within practice transformation and find new innovative and development strategies for PCMH recognition.
  • Learn first-hand from a health center who is seeking recognition going through the 2017 NCQA PCMH standards process.

PRESENTERS
  • Patricia Talbot, RN, BHSA, NCQA PCMH CCE, PCHH Practice Transformation Coach 
  • Ms. Talbot is the Principal and Senior Executive of P.M. Talbot Enterprises. Patricia has over 25 years of progressive positions in health care, which includes health center (community, rural health, FQHC Look-A-Like, and FQHC 330 Grantee), hospital, private practice, and community.
