WEBINAR 1: NCQA PCMH 2017 Standards: A Primer for Health Centers
A patient-centered medical home (PCMH) is a health center that has undergone practice transformation and empahsizes commitement to patient-centered care, population health management, care coordination and ongoing quality improvement. The PCMH care delivery model coordinates patient treatment throuigh their primary care provider to ensure patients receive the necessary care when and where they need it, in a manner they can understand. The appropriate audience for this webinar include staff involved in setting and implementing the quaity trajectory and practice transformation for the organization, including clinical managers.

LEARNING OBJECTIVES
  • Understand processes and procedures that demonstrate transformation into a patient-centered medical home model;
  • Enhance knowledge about acceptable reporting requirements, documentation forms, and how to present it; and
  • Learn about the distinction modules that include a) Patient experience reporting, b) Electronic measure reporting, and c) Behavioral health integration.

Ms. Talbot is  the Principal and Senior Executive of P.M. Talbot Enterprises. Patricia has over 25 years of progressive positions in health care, which includes health center (community, rural health, FQHC Look-A-Like, and FQHC 330 Grantee), hospital, private practice, and community.
