Health Homes Rate Setting Tool for CB-CME Health Centers

The Statewide Quality Improvement Committee (SQIC) Peer Network is pleased to offer this special webinar session to all health centers and other interested stakeholders. As a Peer Network activity, the webinar is offered free of charge.

It is critical for community clinics and health centers contemplating a role as potential CB-CMEs in the Health Homes Program to identify the costs, staffing, and infrastructure needs associated with the new benefit.  John Snow, Inc. has published the Health Homes Rate Setting Tool to support health centers in identifying costs associated with the HHP benefit.  This tool can give health centers a starting point in negotiating rates for Health Homes services with their plans, as well as providing insight into what additional resources and staffing may be needed.  Though this was designed with the Health Homes program in mind, this tool may be useful for health centers considering other care management or care coordination initiatives.

Please join John Snow, Inc. for a review of the rate setting tool, including its practice implications and technical use.  Additionally, we will hear from Harbage Consulting about the state’s training and technical assistance plan for plans and providers.
