Strategies for Effective Hypertension Management & Control in Community Health Centers

Hypertension control remains one of the most critical indicators reported through the Uniform Data System (UDS) and a key driver of chronic disease outcomes in community health centers. This session will provide an overview of evidence informed strategies to improve hypertension management and support sustained blood pressure control across diverse patient populations. Presenters will highlight practical approaches that health centers can adapt within their clinical workflows, care teams, and patient engagement efforts to advance performance on the Controlled Hypertension measure.
LEARNING OBJECTIVES
- Identify key factors that contribute to uncontrolled hypertension and describe their impact on UDS performance for the Controlled Hypertension measure.
- Apply AMA MAPTM Hypertension aligned evidence based strategies and action steps—such as accurate blood pressure measurement, standardized treatment protocols, and partnership with patients to enhance hypertension management practices in their health center.
- Evaluate opportunities leveraging a team-based approach to achieve optimal outcomes for patients experiencing hypertension.

Cancellation Policy
- No refunds for cancellations within 10 business days of the event
- Cancellations before that deadline may receive a 50% refund if submitted in writing to training@cpca.org
