Driving Diabetes Care Forward: Leveraging Data and Performance Improvement Strategies for Clinical Quality

Building on foundational concepts of diabetes program implementation, this webinar focuses on practical, data-driven approaches to strengthen diabetes care quality in community health centers. Participants will explore how to use electronic health record (EHR) data and population health tools to identify patients with pre-diabetes or diabetes, close care gaps, and track progress toward quality improvement goals. The session will provide guidance on implementing programs like the National Diabetes Prevention Program (NDPP) and Diabetes Self-Management Education and Support (DSMES), including foundational coding and billing concepts essential for successful program launch. Through a real-world case study from Chapa-De Health Center, attendees will learn how one health center successfully implemented their NDPP program and achieved measurable outcomes.

- Describe foundational coding and billing concepts necessary for launching diabetes prevention and management programs like NDPP and DSMES.
