DEMO- Thriving in Community Health with Value-Based Care

The COVID-19 pandemic continues to present community health centers with unique challenges to efficiently and effectively providing quality care for their patients and communities. Whether it's identifying and prioritizing patients, improving access and mitigating no-shows, closing quality gaps, or maximizing payer incentives, value-based care represents an opportunity: to focus on patient outcomes and be rewarded for the quality work health centers already do every day. In this practical session, learn how and why community health centers and their patients are thriving in value-based care.

LEARNING OBJECTIVES

• Explore the basics of value-based care, industry trends toward the model, and the natural alignment with community health centers
• Evaluate how and why value-based care improves health center workflows and patient outcomes
• Identify strategies to overcome barriers and obstacles that uniquely hinder health centers from advancing in value-based care
• Implement a concrete action plan to successfully and seamlessly transition your health center into value-based care

HOW TO REGISTER
In order to register for any CPCA event, you must first be logged in to your account. If you do not have an account you can create one. Continue scrolling to the bottom of the page and click Proceed to Checkout. Lastly, complete all payment details and click Submit. You will receive one immediate email with a receipt. CPCA provides training and education to community health centers for general illustration and instructional purposes. Registration is required for all CPCA training and events.You will receive two email communications immediately upon registering. The first is simply a receipt. The second from customercare@gotowebinar.com and will contain webinar access information.  This email will only be delivered to the individual who is registered for the event and will contain a unique link for a single connection. We recommend you add this event to your calendar using the link contained in your webinar access email. If you do not receive this email, please check your spam/junk folders before contacting training@cpca.org.

Cancellation Policy: Effective 11/1/21 - No refunds will be available for cancellations that are received within 10 business days before the start date of the event/training. To receive a maximum refund of 50% of the registration price prior to this deadline, all cancellations must be in writing and submitted by emailing training@cpca.org. Substitutions of registrations in lieu of cancellation is encouraged.
