Regenerative Food as Medicine: Local Health Centers, Local (Regenerative, Organic) Farms

As Food as Medicine (FAM) initiatives gain momentum across California and nationally, community health centers are increasingly exploring how to integrate nutrition, regenerative agriculture, and chronic disease management into primary care. This session highlights the Recipe4Health model, an evidence informed, scalable approach that connects Federally Qualified Health Centers with local regenerative and organic farmers that results in a health multiplier with co-benefits to human health (improve patient health outcomes, address food and nutrition insecurity), economic health (for health systems, agriculture, local economy), and ecological health (improved soil health, decreased greenhouse gases).
Leaders and clinicians from Alameda Health System, Bay Area Community Health, and Tiburcio Vasquez Health Center will share how they implemented FAM programs within busy clinical environments, including screening workflows, EHR integration, staffing models, funding strategies, and lessons learned. Participants will gain practical insights into building cross sector partnerships, aligning priorities, and designing sustainable, equity centered nutrition interventions that support both patients and local growers.
LEARNING OBJECTIVES
- Describe at least three core components of the Recipe4Health model and explain how integrating regenerative/organic agriculture into primary care be a health multiplier for human health (chronic condition outcomes and food insecurity), economic health, ecological health, and equity.
- Identify key operational elements required to implement a Food as Medicine program, including screening workflows, EHR documentation, staffing models, and strategies used by participating health centers to overcome implementation barriers.
- Summarize the role of community or cross sector partners essential for launching and sustaining a Food as Medicine initiative and articulate how these partnerships align with population health and social needs priorities.
