Advancing Quality Together: Promising Practices in Local Plan/Clinic Collaboration

Local health plans and community clinics have long been key partners in providing access to primary care services to California’s Medi-Cal population. Clinics help form the backbone of the safety net in communities across California and work with local plans to reach individuals who might otherwise go without care or seek services in less appropriate or more costly settings.  

An array of external factors impact both local health plans and clinics, including shifts in the demographics of local communities and health care needs of individuals and families; unforeseen health crises, such as the COVID-19 pandemic; systemic changes in reimbursement and payment mechanisms; new and emerging federal and/or state regulatory requirements; and changes in the quality landscape and requirements around quality improvement.    

In collaboration, the Local Health Plans of California (LHPC) Institute and California Primary Care Association (CPCA) will host a two-part webinar series that will discuss the quality improvement landscape, highlight the necessity and value of plan/clinic collaboration, walk through the realities of current performance and explain why and how this matters to plans and clinics.  Presenters will explore the nexus between data sharing and quality, and offer recommendations for promising practices in quality improvement at the plan and clinic level.  Participants will also hear case studies of successful plan/clinic partnerships in pursuit of advancing quality across the continuum of care."


LEARNING OBJECTIVES

  • Describe the current quality landscape and State requirements for plans and clinics
  • Discuss the realities of how and why plan/clinic collaboration is both essential and mutually beneficial, and how such collaboration helps to address challenges and promote improved quality for Medi-Cal beneficiaries
  • Explore the relationship between data sharing and quality improvement
  • Share positive case studies of plan/clinic partnerships and identify strategies to advance quality for both plans and clinics
