CPCA is pleased to host the HRSA Sponsored Region IX NCQA Trainings in partnership with the Arizona Alliance for Community Health Centers, Nevada Primary Care Association, Hawaii Primary Care Association, Pacific Island Primary Care Association, Community Clinic Association of Los Angeles County, and the Redwood Community Health Coalition.
The NCQA PCMH Recognition program plays an integral role in practice transformation and improving the quality of health care. This advanced course is intended for individuals who have applied NCQA PCMH Recognition requirements in the practice setting. It examines the PCMH 2017 standards and the redesigned assessment process. It takes a hands-on approach to each PCMH Concept, and a deeper dive to explore the key characteristics of a successful medical home.
This course will discuss evidence to support the core and elective criteria. It will also focus on advanced topics such as the Distinction Modules, electronic Clinical Quality Measures, and how to receive autocredit. Attendees will participate in facilitated activities that demonstrate how earning elective credits builds upon the Core criteria of a Competency. Our faculty will incorporate topics on behavioral health integration, the importance of assessing the social determinants of patient health, engaging with patients and the community, and continuous quality improvement.
What You Will Learn
At the conclusion of the program, participants will have the knowledge they need to:
- Discuss transfer credit and distinctions.
- Explain how elective criteria build upon core criteria within a competency.
- Examine opportunities and approached to continue to succeed in the PCMH model of care.
- Determine a strategy for preparing a practice for the three (3) virtual check-ins and begin to create a transformation plan.
- Describe the types of evidence a practice can provide to meet the intent of a criterion, competency, and concept.
Who Should Attend
This interactive program is geared towards individuals with significant knowledge of the NCQA PCMH Standards and experience implementing those standards in a practice setting and who have attended the Introduction to PCMH 2017 seminar; and those who intend to obtain the individual Patient-Centered Medical Home (PCMH) Content Expert Certification credential. Registration is limited to community health center staff. Registration is limited to 2 staff members per health center organization. Subsequent registrants from the same health center will be put on a waitlist and be invited to participate if room is available.
When participants register through NCQA's registration link, they receive access to program materials, as well as ensure that continuing education credits as outlined below are awarded following completion of the program evaluation.
In support of improving patient care, the National Committee for quality assurance is jointly accredited by the Accreditation Council for Continuing Medical Education (ACCME), the Accreditation Council for Pharmacy Education (ACPE), the American Nurses Credentialing Center (ANCC), and the Association of Social Work Boards (ASWB) to provide Interprofessional Continuing Education for the Healthcare Team.
Advanced PCMH: Mastering the Medical Home Transformation
This activity was planned by and for the healthcare team, and learners will receive 4.75 Interprofessional Continuing Education (ICPE) credit for learning and change.
This educational activity is approved for 4.75 AMA PRA Category 1 Credit TM, ANA CNE, APA, ACPE, and ASWB ACE.
NCQA PCMH Certified Content Experts (CCE)
Non-CCE participants who complete both the Intro and Advanced PCMH Seminars will be eligible to sit for the CCE Exam. Participants who are already CCEs can earn Other CCE Continuing Education credits for completing the Intro PCMH seminar (8 CEUs) and the Advanced PCMH seminar (6 CEUs). This training is open to staff members from community health centers and is offered at a rate of $50 thanks to support from the Health Resources Services Administration (HRSA), which is a discount from the usual $1,065 fee for this training.
Advance registration is required for all CPCA training and events. In order to ensure you receive all important communications and any materials which may be made available in advance, you must register for events at least on week prior to any event.
To receive a refund, all cancellations must be in writing and must be received by emailing Training@cpca.org according to the following schedule:
- Cancellations received on or before 20 business days of the event will result in a full refund minus a 10% processing fee.
- Cancellations received on or before 10 business days of the event will result in a 50% refund minus a 10% processing fee.
- Cancellations received on or before 5 business days of the event will result in a 25% refund minus a 10% processing fee.
- No refunds will be offered for cancellations received after 5 business days before the event.
- Cancellations after the conclusion of the event are non-refundable.
- Substitutions are encouraged.
- No Shows” are non-refundable
