CPCA is pleased to host the HRSA Sponsored Region IX NCQA Trainings in partnership with the Arizona Alliance for Community Health Centers, Nevada Primary Care Association, Hawaii Primary Care Association, Pacific Island Primary Care Association, Community Clinic Association of Los Angeles County, and the Redwood Community Health Coalition.
This program is an in-depth guide to the NCQA Recognition requirements and the process for achieving PCMH recognition. Recognized practices and clinicians have proved the relevance of the NCQA PCMH standards to the Triple Aim. The 2017 standards emphasize a practice wide commitment to sustaining the PCMH transformation, demonstrating patient care and population health management, care coordination, and ongoing quality improvement.
Practices are rewarded for integrating behavioral healthcare and addressing the impact of social determinants on patient needs. NCQA requirements are positioned to qualify for the Merit-Based Incentive Performance System (MIPS) Clinical Practice Improvement Activities (CPIA) and to support other MIPS requirements under MACRA.
Participants will be able to hear first-hand and ask questions about this exclusive model developed based on the NCQA PCMH model, which supports the state's initiative to improve primary care and promote the Triple Aim: better health, lower costs, and better patient experience.

What You Will Learn
At the conclusion of the program, participants will have the knowledge they need to:
- Identify the core attributes of patient-centered medical homes.
- Identify required documentation and determine how to present it.
- Describe processes and procedures that demonstrate transformation into the medical home model.
- Understand the roles of the NCQA Representative and Evaluator and CCE.
- Examine the more challenging aspects of the requirements in a variety of practice environments.
- Outline the features and functions of the Q-PASS system.
Who Should Attend
This program is designed for any health care professional or health care decision maker interested in getting the inside perspective directly from expert faculty from NCQA in evaluating primary care practice and their ability to become Recognized as a Patient-Centered Medical Home by NCQA. Registration is limited to community health center staff. Registration is limited to 2 staff members per health center organization. Subsequent registrants from the same health center will be put on a waitlist and be invited to participate if room is available.
Continuing Education
When participants register through NCQA's registration link, they receive access to program materials, as well as ensure that continuing education credits as outlined below are awarded following completion of the program evaluation.
In support of improving patient care, the National Committee for quality assurance is jointly accredited by the Accreditation Council for Continuing Medical Education (ACCME), the Accreditation Council for Pharmacy Education (ACPE), the American Nurses Credentialing Center (ANCC), and the Association of Social Work Boards (ASWB) to provide Interprofessional Continuing Education for the Healthcare Team.
Introduction to PCMH: Medical Home Foundational Concepts
This activity was planned by and for the healthcare team, and learners will receive 7.75 Interprofessional Continuing Education (ICPE) credit for learning and change.
This educational activity is approved for 7.75 AMA PRA Category 1 Credit TM, ANA CNE, APA, ACPE, and ASWB ACE.
NCQA PCMH Certified Content Experts (CCE)
Non-CCE participants who complete both the Intro and Advanced PCMH Seminars will be eligible to sit for the CCE Exam. Participants who are already CCEs can earn Other CCE Continuing Education credits for completing the Intro PCMH seminar (8 CEUs) and the Advanced PCMH seminar (6 CEUs). Cost
This training is open to staff members from community health centers and is offered at a rate of $100 thanks to support from the Health Resources Services Administration (HRSA), which is a discount from the usual $1,250 fee for this training.
Advance registration is required for all CPCA training and events. In order to ensure you receive all important communications and any materials which may be made available in advance, you must register for events at least on week prior to any event.
CANCELLATION POLICY
To receive a refund, all cancellations must be in writing and must be received by emailing Training@cpca.org according to the following schedule:
- Cancellations received on or before 20 business days of the event will result in a full refund minus a 10% processing fee.
- Cancellations received on or before 10 business days of the event will result in a 50% refund minus a 10% processing fee.
- Cancellations received on or before 5 business days of the event will result in a 25% refund minus a 10% processing fee.
- No refunds will be offered for cancellations received after 5 business days before the event.
- Cancellations after the conclusion of the event are non-refundable.
- Substitutions are encouraged.
- No Shows” are non-refundable
