As health centers transition to primarily virtual visits, they need to redefine the care model for chronic physical or behavioral health conditions that captures necessary clinical care and quality indicators. Join CPCA's Care Transformation Team and Community Health Solutions for a moderated discussion about strategies health centers are using to capture clinical care and quality measures in either an all virtual or hybrid (part virtual, part in-person) models. Come prepared to discuss how your health center is capturing measures virtually, what's working, and what you are struggling with.
