As the shift from volume to value-based payment accelerates, primary care providers, including health centers, must make essential changes to become ready for value-based payments and ensure their financial sustainability. But what changes need to be made? And if so, when should you make those changes?

In this 60-minute webinar, participants will hear from two industry experts, Aledade and CalHIPSO, about different options for value-based care models including the ACO model. In addition, we will discuss necessary changes and best practices for risk coding and stratification that health centers should make in order to succeed in a value-based payment environment.

LEARNING OBJECTIVES

  • Provide a high level overview of value-based care and current models of value-based care, including ACO;
  • Deep dive into what it means to be a risk-based organization, hidden issues with improper risk coding, and how to risk-stratify patient populations; and,
  • Demonstrating how health centers transform their coding practices to take on risk.

PRESENTERS
  • Mat Kendall, Executive Vice President Provider Networks, Aledade
  • Michael Lubin, Vice President Growth, Aledade
  • DeeAnne McCallin, Program Director, CalHIPSO
