Remote Patient Monitoring for Hypertension Control: Strategi

Studies have shown the benefits of remote patient monitoring (RPM) programs for monitoring patients with hypertension (HTN) (high blood pressure) outside the clinic. RPM technology allows patient blood pressure readings and other health data to reliably flow between the patient and their care team providers, with advantages that include detection of white coat and masked hypertension and better long-term blood pressure control. With so many RPM programs available since the pandemic, however, choosing the best program for a health center and their patient population can be a challenge. In this webinar, we have heard from health centers who have successfully implemented an RPM program using different RPM technologies, to improve hypertension control rates in patients with hypertension. Presenters discussed considerations for program planning, challenges, successes, and strategies for successful RPM program implementation to improve hypertension outcomes in the health center setting.
