Patient-Centered Health Home (PCHH) transformation is essential but challenging work. The California Primary Care Association (CPCA) is here to support our member clinics by providing technical assistance and training in a wide variety of formats to meet their exact needs. The purpose of the initiative is to create an integrated strategy to support community clinics and health centers (CHCs) to achieve patient centered health home recognition status, improve health outcomes, and position CHCs for ongoing participation in primary care delivery under health reform.

Had it not been for our CPCA Practice Coach, review and suggestions, we would not have achieved our NCQA PCMH Level Three Recognition. Our coach reviewed our documentation to ensure its appropriateness for the Standard, Factor and Element for which it was being submitted. Now that we are recognized, our coach continues to work with Clinicas de Salud del Pueblo to facilitate our PCMH Transformation process.
