Telehealth has been an effective tool for years in treating patients remotely, and as we look to the future, Remote Patient Monitoring (RPM) will continue to grow in popularity as a part of telehealth solutions. Now, see how these solutions can be used in parallel with care management services to improve the health and increase satisfaction for your patients and providers alike in a cost-efficient and outcome-driven way. We will cover RPM and telehealth workflows, and how health centers can use new billable codes to improve health outcomes for high-risk patients with chronic conditions.

LEARNING OBJECTIVES
  • Understand current and updated care management services (eg. TCM & PCM)
  • See RPM workflow outlines for Community Health Centers (FQHCs)
  • Understand Uses for FY 2021 Supplemental Funding for Hypertension (NHCI-HC)
  • Learn how to improve health center margins through telehealth services

PRESENTERS
  • Benjamin Lefever, Founder and CEO, Certintell
