Creating a Statewide Network of Clinics for Individuals Returning from Incarceration

The Transitions Clinic Network (TCN) works with health centers to implement the TCN model of care - a cost-savings, evidence based program that improves health and reentry outcomes among chronically ill individuals returning from incarceration. In this webinar, we will provide background on the impact of incarceration on health, introduce the TCN program, and highlight our initiative to expand the TCN program to primary care clinics in communities impacted by incarceration throughout California. The audience will also hear from a health center who has participated in the TCN for several years, about the successes they have achieved and challenges they have overcome to integrating the TCN model for patients.

  1. Participants will be able to list at least 2 reasons why individuals returning to the community from incarceration have different health needs than individuals who have not been incarcerated.
  2. Participants will be able to identify at least 2 strategies that contribute to improved health outcomes for formerly incarcerated patients.
  3. Participants will be able to identify different mechanisms for funding a community health worker (CHW).
  4. Participants will learn how to apply for the next cohort of TCN health centers in California.
