Developing Care and Services for Older Adults at CHCs

Aging is a known risk factor for many chronic diseases (e.g., diabetes, dementia, and cancer) as well as life-threatening infections (e.g., COVID-19). California’s community health centers are increasingly caring for an aging and more complex patient population. With increasing challenges of health and aging, alongside workforce and training limitations—health centers face a common challenge to improving wellbeing. In this webinar, Asian Health Services is highlighted to showcase the focus of care and resources needed for older adult patients to live healthy and full lives. An overview of AHS’ Integrated Behavioral Health program alongside technological considerations is provided to observe partnerships and potential responses to proactively managing the health and well-being of patients as they age.

 

LEARNING OBJECTIVES

  • Discuss the unique considerations for healthy aging and factors that affect wellbeing for older adult patients at community health centers including social needs, health assessments, and recognition of technological barriers
  • Outline early steps of engaging multi-disciplinary staff and external partners in a comprehensive overview of community resources
  • Identify innovative practices for improving quality of daily life and social supports for older adults
