Innovation, Digital Health, & Partnerships for Hypertension Control

Hypertension (HTN) is a risk factor for cardiovascular disease (CVD) and stroke, both of which are leading causes of death in the United States. Yet, hypertension is the most modifiable risk factor for CVD. Nearly half of U.S. adults (~46%, or 116 million) have hypertension. Achieving HTN control often requires a systems approach that includes patient, provider, care team, and, at times, community engagement for care optimization. In this webinar, we will hear promising practices to achieve hypertension control amongst health center patients. The presenters will share approaches to engaging patents, care teams, and communities to improve hypertension control rates. Promising practices shared will include ACTIVATE, a digital health platform for remote patient monitoring implemented with health coaching and digital literacy, and community-based partnerships for achieving hypertension control.

This lunch and learn webinar is part of a 3-part webinar series.

  • Describe how virtual visits and remote patient monitoring can improve health outcomes for people with diabetes and hypertension in community health centers.
  • Understand the barriers to digital health for chronic illness facing community health centers and their patients and discuss potential strategies to address these barriers.

CPCA provides training and education to community health centers for general illustration and instructional purposes. Registration is required for all CPCA training and events. You will receive two email communications immediately upon registering. The first is simply a receipt. The second from zoom.us, under CPCA Training or a CPCA staff member's name, will contain webinar access information.  This email will only be delivered to the individual who is registered for the event and will contain a unique link for a single connection. We recommend you add this event to your calendar using the link contained in your webinar access email. Cancellation Policy:
Effective 11/1/21 - No refunds will be available for cancellations that are received within 10 business days before the start date of the event/training. To receive a maximum refund of 50% of the registration price prior to this deadline, all cancellations must be in writing and submitted by emailing training@cpca.org. Substitutions of registrations in lieu of cancellation is encouraged.

Invoicing/Pay Later:
Effective 12/1/21 - the "Pay Later" option will no longer be supported when registering for trainings and events that have a fee associated with them. Payment will be required at the time of registration via credit card.
