Enhancing Healthcare Data Collection: Lessons from Implement

Enhancing Healthcare Data Collection: Lessons from Implementing Z-Code Integration in Community Health Centers

DATE:
Wednesday, January 17th, 2024
TIME:  10am to 11am

This presentation focuses on efficient healthcare data collection in community health centers, highlighting the implementation of Z-Code integration, specifically the ICD-10-CM diagnosis codes (Z55-Z65) related to Social Determinants of Health (SDOH). These codes document essential factors like housing, food insecurity, and problems related to psychosocial circumstances. In today's healthcare landscape, accurate data is pivotal for patient-centered care, effective partnerships, and informed policymaking. The session explores the integration of SDOH z-code data to address these challenges. Attendees will gain insights into the hurdles faced by community health centers and their lessons learned regarding SDOH data integration.

Additionally, the presentation underscores the critical role of consistent SDOH data collection, emphasizing its importance within the Population Health Management (PHM) initiative of the California Advancing and Innovating Medi-Cal (CalAIM) program. CalAIM, introduced by the Department of Health Care Services (DHCS), aims to enhance health outcomes and the quality of life for Medi-Cal beneficiaries. Specifically, the PHM initiative identifies and manages member risk and need through comprehensive care approaches while focusing on SDOH. Participants will learn how efficient data collection, particularly related to SDOH, contributes significantly to the success of CalAIM's PHM initiative, shaping a more holistic and effective.
