Integrated Care for Patients With Diabetes & Prediabetes

Colorado Coalition for the Homeless, a federally-qualified community center in Denver, CO, is pilot testing an effort to integrate diabetes prevention and management and oral health in their medical and dental clinics. This 60-minute webinar will describe the Coalition's partnership with the state health department and primary care association and outline the clinic's specific process to advance integration for diabetic patients in terms of workflow, health information technology, data-driven quality improvement, and patient and staff education.

LEARNING OBJECTIVES
  • Enhance understanding of the critical role of partnerships and collaboration to promote integrated care for patients with diabetes; 
  • Improve ability to identify promising practices and barriers for integrating oral health and primary care for patients with diabetes and prediabetes that can be translated to participants' organizations; and
  • Increase knowledge of commonly-used quality improvement strategies that Community Health Centers can apply to their own integration efforts.

Cancellation Policy:  No refunds will be offered for cancellations received after Tuesday, July 24, 2018.

To receive a refund, all cancellations must be in writing and must be received by emailing training@cpca.org according to the following schedule:

  • Cancellations received on or before 20 business days of the event will result in a full refund minus a 10% processing fee.
  • Cancellations received on or before 10 business days of the event will result in a 50% refund minus a 10% processing fee.
  • Cancellations received on or before 5 business days of the event will result in a 25% refund minus a 10% processing fee.
  • No refunds will be offered for cancellations received after 5 business days before the event.
  • Cancellations after the conclusion of the event are non-refundable.
  • Substitutions are encouraged.
  • “No Shows” are non-refundable.
